The tear trough, also known as the nasojugal groove, is a natural depression extending inferolateral from the medial corner of the eye to approximately the midpupillary line. It is seen in some young people and becomes progressively more pronounced with aging. When this external landmark becomes noticeable or unsightly, it is regarded as the “tear trough deformity.” The lid-cheek junction or the palpebromalar groove is located at the lateral lid-cheek junction based on the midpupillary line, below and approximately parallel to the lower orbit. These changes impart a tired appearance despite adequate rest and prompt patients to seek treatment with dermal filler in the periorbital area. The tear trough treatment remains one of the most difficult treatments to execute correctly due to the complex local anatomy and a strict requirement that the filler is placed deep in the correct plane in order to avoid swelling and other complications.
